Membership Terms
All Performance Hyperbaric memberships require a minimum six-month commitment. By enrolling in a membership, clients agree to maintain their membership for the full six-month term.
Membership payments are non-refundable and may not be canceled or terminated prior to the completion of the six-month period. If a client chooses to cancel their membership before the end of the initial term, the full remaining balance for the six-month commitment will become immediately due and payable.
Performance Hyperbaric understands that unforeseen circumstances may arise. If a member is unable to utilize their sessions for any reason, the remaining services may be transferred to another person with prior written approval from Performance Hyperbaric. All transfers must comply with current membership policies and are subject to administrative review and consent to ensure proper usage and scheduling.
These terms are designed to comply with all applicable state and federal laws governing membership and service agreements. They are intended to ensure fairness, continuity of care, and scheduling integrity for all clients.
Following the completion of the initial six-month term, memberships may be renewed, modified, or canceled in accordance with the terms outlined in the client’s individual membership agreement.
